Life by
Design was incorporated in July 1998 with the idea that the provider can be a
partner or a hindrance to people who want to live a self-determined life – we
chose to be a partner. We provide
support, one person at a time. Each
person’s situation is unique and we invest the time to listen and understand
what is really wanted and needed. When
a plan is well thought out it can bridge strengths and resources so that a
person can get on with his or her life. Using
this approach has allowed us to grow to support over 100 children and adults in
leading self-determined lives.
Life By Design, Inc. offers support
to individuals with disabilities through Home and Community based Waivered
services including MRRC and CADI waivers as well as SILS and other similar
services. We serve children living
with their families as well as adults living in their own apartments.
We offer supported employment so that people can find real jobs in their
community. Our services are
currently provided in five counties, Anoka, Hennepin, Isanti, Pine, and
Sherburne counties.
Life
By Design currently employees 200 direct support professionals who provide
supports for people at home, work, or in the community.
We have an established comprehensive orientation program that meets
licensing requirements and establishes a solid foundation for providing quality
supports. We recognize the
significant impact that experience and employee education makes on the quality
of supports and services.
Our
staff’s experience within the agency is grounded in the principles of
self-determination. When
services are guided by self-determination, the experiences shared with consumers
and their allies are always interesting. We
strive to employ support professionals who share our commitment to people with
disabilities, discerning their own path in life, reaching personal life goals.
Our staff brings creativity, competence and a willingness to think
outside of the box.
